Evidence lifecycle

Assess readiness at the gate where the work actually stands.

Every stage begins unassessed. A reviewer assigns a state only after inspecting scope-appropriate evidence.

Discovery · state: not assessed

Inspect decision need, scope, roles, data boundary, current evidence, assumptions, dependencies, risks, acceptance conditions, and stop criteria.

Prototype · state: not assessed

Inspect the highest-risk relationship through a bounded scenario; record observed behavior, limitations, defects, accessibility or control questions, and disposition.

Pilot · state: not assessed

Inspect representative roles, operating cadence, ownership, data, exceptions, support, measures, and the evidence needed for scale, adjust, hold, or stop.

Production gate · state: not assessed

Inspect traceability, security, privacy, accessibility, performance, UAT, defects, support, recovery, rollback, release authority, and residual-risk ownership as applicable.

Operate or exit · state: not assessed

Inspect monitoring, complaints, support, integration health, recovery, improvement decisions, export, transition, deletion, and accountable exit responsibilities.

Scorecard rule: no stage is prechecked; record definition needed, evidence partial, ready, not applicable, or unresolved only from reviewable evidence.
